Medical Records Coordination

In accordance with HIPPA regulations, North Cascade Cardiology safeguards our patients' personal health information. Patients do have the right to access and receive copies of their medical records. Patients wishing to do so must sign and date an official release form and return the form to our office before records are released.

In addition to hard copies of medical records, North Cascade Cardiology is also pleased to offer our patients records via MyRecord, a new technology that provides access to patient medical records in the event of an emergency medical situation. MyRecord is a credit-card sized compact disk that contains important medical information, pulled directly from patients' medical records at our practice. The disk fits easily into a wallet or purse and allows patients to carry health records with them at all times. Information included on the MyRecord disk:

  • Current Medication List
  • Current Diagnosis
  • Current Allergies
  • Hospital Discharge Summaries and Office Visit Reports
  • Diagnostic Image Reports (Ex: echocardiograms)
  • Emergency Contact Information, Insurance, and Physician Contact Information
  • Reference and Education Materials (specific to patient's diagnosis and medication list)

Patients may order MyRecord while in clinic by signing a release form specific to MyRecord and paying a nominal fee. Disks are then mailed directly to the ordering patient's home address within thirty days.

Note: North Cascade Cardiology encourages patients to update their MyRecord information disk annually, as new information cannot be added to existing disks.
